A lot of new comers will ask the question
" Where I can find the Immuzation form in the school website?"
and Here is the web-address

All you have to do is download the International Student Immunization Form.
Go to hospital where provides health check, let the doctor finish this form.
(maybe you have to need the needles! Confused

)
Then, bring this Form to Miami. ( Don't forget)
Hand in the Form in Health Center.
That's it!! Smile

...Guys!! Don't forget the form!!! It is very important!! Make sure the information is completed!!! Otherewise, it will cost you 40 dollars to take each shot(you will need 2) and 20 dollars to do the blood test!! Save your money!! Be prepared!!! Wink
